Mandated Testing

Third Party Test Witnessing: As the third-party witness agency, we propose to perform

the following Category Test Witnessing services in compliance with testing

procedures mandated by the New York City Administrative Code.

Category 1 Test (For Elevators, Escalators, and other qualifying devices):

  • Schedule and coordinate inspection/test with elevator contractor and management, as well as provide schedule notice to the NYC Dept. of Buildings in advance, as applicable (10 days notice for escalators). Provide copy of the testing schedule to management, upon request.

  • Witness the annual “Category 1” no load annual tests/inspections [to be performed by a qualified elevator company retained by owner/management], as required by the NYC Dept. of Buildings. 

  • Witnessing Inspector and Agency Director shall sign test reports, as required. Prepare required test/inspection reports (ELV-3 forms), and process with the NYC Dept. of Buildings, as required (along with required filing fees, as applicable) within the required time frame.

  • Provide checklist of any deficiencies noted during testing to be corrected by Elevator Contractor.

  • Provide copy of test reports to owner/management for record purposes.

  • Review Dept. of Buildings’ records to ensure that test reports (filed by our office) were properly posted in the system in order to avoid violations.  If warranted, submit copies of “stamped” reports as proof of proper and timely filing in order to cure any erroneous testing violations.

  • Review any repair proposals submitted by the assigned Elevator Contractor to cure deficiencies noted during Category testing that may not be covered under existing service agreement; and negotiate quoted fees, as warranted.

 

Category 5 Test (For Elevators Only):

  • Review property records to verify the due date for the Category 5 tests (on a per device basis).

  • Prior to scheduling test, meet with management to discuss need for additional time due to counterweight safeties (where applicable).

  • Coordinate and schedule test with the elevator contractor and management (test weights required). Schedule test with the NYC Dept. of Buildings, as required (with 7 days notice).

  • Witness the “Category 5” test [to be performed by a qualified elevator company retained by owner/management], as required by the NYC Dept. of Buildings. 

  • Witnessing Inspector and Agency Director shall sign test reports, as required.

  • Prepare required test reports, and process with the NYC Dept. of Buildings, as required, along with applicable filing fees.

  • Provide copy of test report to management for record purposes.
